Mishka's Death: Following Procedure Could Have Prevented Incident

The Ministry of Health’s report on the death of 10-year-old Mishka, publicized on Sunday, stated that the incident was improbable if those attending to the episode had followed proper procedure.
The report claimed that treatment was delayed due to staff at the Health Emergency Operations Center (HEOC) and the medical response team failing to identify the situation as an emergency.
Call logs showed that the center was unaware of the seriousness of the 10-year-old’s condition. The report further stated that the ambulance was not deployed as the doctor working with the medical response team did not know it was an emergency. Instead, a message was sent to the clinical medical response team for additional consultation.
The doctor attending had neglected his obligations to those needing medical attention, the ministry noted.
The family had called for online consultation in the afternoon when Mishka’s health began to decline. The doctor on call failed to advise a physical assessment even after noting her condition.
Later that night, the doctor called the family and informed the Clinical Management Advisory Team (CMAT) of the situation, but did not convey it was an emergency. When the parents called due to the ambulance’s delay, the call center did not confirm whether an ambulance was arranged or dispatched.
The report read that the call center, in this scenario, did not follow new standard procedures to update the Disaster Response Team (DRT).
Research showed that the call center did not use HEOC’s system to update the status of the cluster.
The system was developed to manage the inflow of information but did face interference due to several reasons, including internet speeds.
The report also highlighted that DRT had a lack of specialist doctors.
